EPIDEMIOLOGIAL STUDY OF PARAGONIMIASIS IN YANGPYUNG-GOON, KYUNGGI-DO

Abstract
Yangpyung-Goon is located in the eastern Part of Kyunggi-Do and borders on Kangwon-Do on the east and north east, with Kapyung-Goon , Kyunggi-Do on the north, with Yanjoo-Goon and Kwang-joo-Goon on the west. with Yujoo-Goon, Kyunggi-Do on the south. Yaugpyun9-Goon is mountainous and contains twelve Myuns . Yangpyung-EuP, Kae-koon-Myun, Suhjong-Myun, Danwol-Myun, Yong-moon-Myun, Kangsang-Myun, Kangha-Myun. Yangsuh-Myun, Okchun-Myun, Jije-Myun and Chvngwoon-Myun. Each Myun has about 10,000population and has 3 to 7 Ris.
This survey was directed to this Goon as a part of the program of studying directly making a trip to each Myun in every Goon in Kyunggi-Do as finding out the districts which have the poisonous crayfish and crabs having Paragonimus metacercaria is urgent because children in rural are as eat raw and slightly roasted crayfish and crabs and there is a traditional custom in Korea of taking raw crayfish juice for the treatment of measles, producing many public health problem.
Among twelve Myuns in Yangpyung-Goon. twoMyuns. Yangpyung-EuP and Kangha-Myun, are excluded because the former is consistedof big town and is considered to be not so infested by the Paragonimus westermani and the latter is considered to get similar results to that of Kangsang-Myun which borders on Kangha-Myun and has simitar situations. One primary school from each Myunin the other ten Myuns was selected and the Pupilsin older age groups were tested for this survey being asked about their custom of eating raw and inadeqllately cooked crayfish and crabs and theireavironmental situation regarding drinking water. and testing them with Paragonimus antigen. The cray fish caught from these Myuns were examined for Paragonimus metacercaria.
The children ate raw crayfish were ranged fromnone in Kaekoon-Myun to 32.3% in Chungwoon-Myun, over 50% of male children and few femalechildren in many Myuns in this Goon ate crayfish, the children who ate poorly cooked crayfish wereranged from 35.0% in Kaekoon-Myun to 95.9% in Yangsuh-Myun. Such eating habits among childrenin this Goon are suggestive of a large in festationand of a continuation of paragonimiasis from early childhood. In environmental situations, those whodrink the water of streams and from shallow well shave possibility of acquiring Paragonimus metacercaria from the water directly. Those children whodrink such water were ranged from 17.2% in Dan-wol-Myun to 71.1% in Suhjong-Myun, showing over 50% in Yongmoon-Myun. Kangsang-Myun,Yangsuh-Myun, Yangdong-Myun, Jije-Myun,Chungwoon-Myun and Suhjong-Myun.
The positive rate with Paragonimus skin tests show 1.3%-6.8% in such Myuns having lowermountains and fields as Kaekoon-Myun. Kangsang-Myun, Jije-Myun, and 8.3% in Yangsuh-Myun, 13.0% in Okkchun-Myun, 18.3% in Yangdong-Myun 21.1% in Danwol-Myun, about 45% in suhjong-Myun, Chungwoon-Myun and 67.6% in Yongmoon-Myun, which is highest percentage in this Goon and even in whole Kyunggi-Do.
This Goon is very mountainous and in the many valleys in the deep mountains, the first intermediate host, snails, the second intermediate host, crayfish, propagate actively and ae produced in enoumous numbers. Crabs are present in the limited season from August to November and in the limited areas located along the big river. North Han River and Han River, and are not likely to be a second intermediate host of paragonimiasis in this Goon. The infestation rate of crayfish with Paragonimus metacercaria was found to be as follows:
1.9% in Suhjong-Myun, 1.4% in Danwol-Myun, 4.5% in Yongmoon-Myun, 0.8% in Yangsuh-Myun, 1.3% in Okchun-myun, 0.6% in Yangdong-Myun, 2.7% in chungwoon-Myun respectively.
The sustom of taking raw crayfish juice for the treatment of mealsles is also prevalent in this goon which may give many harmful effects to growing children. I wigh the public health authorities and the school authorities in this Goon would strive to eradicate this bad custom.
